Home Medication Reviews

Managing multiple medicines can be complex — particularly when health conditions change, new medicines are added, or care is shared across different providers.

A Home Medication Review (HMR) provides an opportunity to step back, review the full picture and ensure medicines are working for you, not against you, supporting safe, effective and confident medication use.
